Job Details: Summary: Using telephonic interaction as the mode of delivery, facilitates access to care by assessing needs, offering clinical options, and referring to health care providers Provider of after hours medical management for patients of Cleveland Clinic. Serves as a clinical contact for hospital patients post discharge. Ability to therapeuticly interact with a variety of patient poulations. Able to work in a fast paced environment commited to providing immediate resolution to patient problems. Job Responsibilities: Elicits, assesses and prioritizes patient information and recommends most appropriate level of care using evidenced based protocols. Exhibits the ability to adhere to established guidelines, employ critical thinking, satisfy patient needs. Collaborates with all health care providers to optimize patient outcomes. Provides after hours care to various pateint populations in a variety of situations. Documents patient encounters in electronic medical record. Responds to caller's requests for a referal to a physician or for health information from approved sources. Provides physician referrals to appropropriate specialists. Exhibits a workable knowledge of the teaching principles of health care information to pediatric, adolescent, adult and geriatric populations. Maintains professional growth and development. Demonstrates an active participation in education. Maintains a current knowledge base, including triage skills and decision support tools, teaching principles, departmental processes and resources. Completes all department and organizational annual competencies as assigned. Accomplishes personal goals as outlined in Individual Development Plan. Consistently conveys a positive, empathetic,and therapeutic rapport in all workplace encounters.Responds to change in a positive manner. Interacts with callers, physicians, other departments, and member of the community in a professional and respectful manner. Demonstrates respect for individual, cultural and social diversity. Maintains confidentiallity of information. Displays a commitment to the Patient First Philosophy. Other duties as assigned. Education: Demonstrated excellent written and oral communication skills. Active listening skills. Ability to adhere to clinical guidelines and exhibit critical thinking skills in stressful encounters. Proficiency in Windows based applications, and ability to navigate efficiently. Experience: Minimum 3 years experience with demonstrated competence in all aspects of nursing care and independent decision making. Experience in telehealth triage, emergency medicine, adult/pediatric ambulatory care, home health care preferred. Licensure/Certification/Registration: Graduate of an approved school of nursing. Current license in State of Ohio. CPR certified. Current demonstrated clinical proficiency. Telehealth Triage or Ambulatory Nursing Certification a plus. Physical Requirements: Ability to sit for long periods of time within a confined space. Requires full range of motion, manual and finger dexterity of eye hand coordination. Requires corrected vision and hearing to a normal range. Sedentary Work – Exerting up to 10 pounds of force occasionally (Occasionally: activity or condition exists up to 1/3 of the time) and/or a negligible amount of force frequently (Frequently: activity or condition exists from 1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body. Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
