Shift/Schedule: Full Time Position: Primarily: 1st Shift 7AM - 3:30 PM. Including weekend and/or holiday rotation.

Summary:
Licensed Social Worker (LSW) or Licensed Professional Counselor (LCP) in the State of Ohio. Social Worker or Case Management Certification within one year of eligibility. Master's Degree in Social Work required. Knowledge of NASW Code of Ethics practices. Indirect supervision of a LISW, coordinates care and services of select patients across the continuum of illness. Promotes effective utilization of health care resources to achieve optimal clinical and resource outcomes. Conducts needs assessments and provides information and referral with community resources. Hospital Social Work and Oncology experience preferred. Exposure and/or experience in pre-acute and post-acute care; knowledge of available community resources preferred.
