Summary: Evaluates and treats home health patients with physical, emotional, cognitive and/or psychosocial deficits which impact their abilities to successfully perform independent living skills. Job Responsibilities: Assesses patient's ability to safely and effectively perform independent living skills. Designs the appropriate plan of care, including type and frequency of treatment. Implements the treatment plan using appropriate modalities. Documents subjective and objective findings of patient progress. Monitors and modifies the plan of care as needed. Seeks consultation as necessary. Educates the patient and family/caregiver about patient deficits. Plans and/or assists with patient discharge from therapy services and treatment setting. Provides information regarding appropriate selection/use of adaptive equipment and support programs. Oversees the care provided by certified occupational therapy assistants and therapy technicians according to established licensing laws, state regulations and OT practice standards. Other duties as assigned. Education: Bachelor's degree required. Must be a graduate of an Occupational Therapy Program accredited by the American Occupational Therapy Association Experience: At least 1 year clinical experience as a licensed occupational therapist in an acute, rehab or skilled nursing setting. Licensure/Certification/Registration: Current Occupational Therapy (OT) licensure in the State of Ohio. Current Cardio Pulmonary Resuscitation (CPR) certification. Valid Ohio Driver’s license (VDL). Current auto insurance $100,000/$300,000 coverage. Physical Requirements: Must be able to travel to patient’s home. Must be able to climb stairs, pull, push and facilitate movement of the patient. Must regularly lift and/or move up to 25 pounds, frequently lift and/or move up to 50 pounds and occasionally lift and/or move more than 100 pounds. Must be able to transport laptop computer and/or necessary medical/patient equipment and supplies. Must have normal or correctable vision. Must have normal or correctable hearing to be able to hear normal conversations. Must be able to work in settings with infectious diseases. Must present a professional appearance at all times.
