Job Details: - $100,000/300,000 auto insurance coverage

- Valid OH Driver's License
- 1-2 years home care experience preferred
- 2 years clinical experience required
Summary: Initiates and/or resumes home care after verification of patients’ eligibility for home care services. Job Responsibilities: Conducts the comprehensive assessment, including OASIS data collection and develops the plan of care to meet immediate and ongoing care and support needs. Provides ordered treatments and patient/caregiver education in accordance with agency policies and procedures and the Conditions of Participation Identifies immediate care and support needs and obtains orders to address psychosocial and treatment priorities. Makes appropriate admission decisions based on patient eligibility, acuity and safety. Updates and confers with the physician to establish an individualized plan of care and facilitate achievement of expected outcomes. Prioritizes and documents patient problems based on assessed needs/problems in collaboration with patient, family and health care team Assesses educational needs / limitations of patients and significant others with consideration for language barriers, mental and cognitive status, cultural characteristics, and other pertinent findings. Provides teaching and educational material to the patient Conducts review of the medication regimen and collaborates with appropriate agency resources and physicians as indicated. Demonstrates competence in physical and psychosocial assessment skills and compliance with documentation of established assessment parameters. Addresses patients’ coverage/financial concerns through education of financial rights/responsibilities, Medical Social Work consultation, application for financial assistance, communication with Insurance Case Managers and billing department, as indicated Evaluates and appropriately identifies the need for outreach community services or other agency services. Other duties as assigned. Education: Bachelor's Degree Experience: Minimum 2 years clinical experience (strong medical/surgical background); 6 months home health experience preferred Licensure/Certification/Registration: Current licensure as a Physical Therapist in the State of Ohio. Curent CPR certification. Valid Ohio Driver’s license upon hire; per policy 8-175 thereafter. Proof of automobile insurance coverage with $100,000/$3000,000 coverage, upon hire; per policy 8-175 thereafter Fulfill requirements for: Certificate for OASIS Specialist-Clinical, within one year Physical Requirements: Must be ambulatory and able to drive to patients’ homes and potential customers, able to climb stairs, pull, push, and facilitate movement of the patient. Must regularly lift and/or move up to 25 pounds, frequently lift and/or move up to 50 pounds and occasionally lift and/or move more than 100 pounds. Must be able to read, write and communicate with patients .Must have correctable vision and correctable hearing to be able to hear normal conversations. Must be able to work in settings with infectious diseases. Must present a professional appearance at all times and attend staff and agency meetings, as requested.
