Summary: Under the direction of the Director, Patient Education & Health Information. Performs patient activities involving education and counseling sessions, liaison function for physician consults,coordination of various patient educational activities.
Job Responsibilities: Conducts education and counseling sessions for patients and significant other(s) in order to enhance disease/condition control and treatment compliance. Provides the patient and family with appropriate patient education material necessary to carry out prescribed treatment. Consults with physicians, head nurses, case managers, primary nurses dietitians, social workers, alternate site planners, etc., regarding recommendations for patients treatment. Assists patients and significant other(s) with appropriate educational follow-up. Provide patients, employees and visitors with information to promote health and disease prevention. Assists in coordinating home plans with nurses, physicians, alternative site care coordinator, Kaiser outpatient educator, etc. Participates in appropriate educational committee (e.g., Patient Education Material Committee, Patient Education Activities Committee, etc.). Documents educational session in medical chart to inform referring physician and other health care professionals the teaching session results (including recommendations for home care, medication changes and patient needs). Develops educational standards for all patient instruction sessions. Assists in the update research implementation and evaluation of patient educational programs and/or literature. Provides on-site health information to patients, employees and visitors for the Health Information Library. Assists with organizing various health programs, lectures, workshops, community seminars, health fairs, etc. Coordinates the installation and maintenance of the TV schedule wall holders in the hospital rooms. Other duties as assigned
Education: Bachelors Degree in appropriate allied health field, e.g., Health Educator, nursing or related area. Masters Degree preferred.
Experience: Three years of experience in an educational environment or related field. Thorough understanding of teaching methods, learning theories and program development (assessment, planning, implementation and evaluation). Health Sciences background would be helpful to understand medical contents.
Licensure/Certification/Registration: BSN,RN,CDE for diabetes teaching only.
Physical Requirements: Manual dexterity sufficient to perform computer functions and above duties.
