GENERAL SUMMARY: Responsible for providing physical therapy services to assigned patients including evaluation, treatment planning, treatment programs, patient/caregiver education and discharge planning in accordance with the agency policies and procedures and the physician care plan. Performs admissions and assumes responsibility for managing patients; maintains timely communications with others involved in the care of the patient. Provides supervision to physical therapist assistants, home care aides and family/caregivers involved in the care of the patient. Participates as a member of a patient -focused interdisciplinary care team.
MINIMUM QUALIFICATIONS:
Education, Knowledge, Skills and Abilities
Graduate of an accredited Physical Therapy program.
Must be able to travel to patient’s home.
Must be able to read, write, and communicate with patients.
Must be able to work in settings with infectious diseases.
Must present a professional appearance at all times.
Required Length and Type of Experience
At least 1 year clinical experience as a licensed physical therapist in an acute, rehab or skilled nursing setting.
Required Licensure, Certification or Registry
Current licensure in the State of Ohio.
CPR Certification
Valid Ohio Driver’s license upon hire; per policy 8-175 thereafter. Proof of automobile insurance coverage with $100,000/$300,000 coverage, upon hire; per policy 8-175 thereafter.
Required Physical and Environmental Demands
Must be able to climb stairs, pull, push and facilitate movement of the patient.
Must regularly lift and/or move up to 25 pounds, frequently lift and/or move up to 50 pounds and occasionally lift and/or move more than 100 pounds.
Must be able to transport laptop computer and/or necessary medical/patient equipment and supplies.
Must have correctable vision.
Must have correctable hearing to be able to hear normal conversations.
