Summary: Facilitates cost-effective, quality patient outcomes by collaborating with staff and providing guidance, education and support to assigned clients.

Job Responsibilities: Collaborates and consults with multidisciplinary team regarding clients progress. Performs continuous assessments and evaluations to monitor client's progress. Works with interdisciplinary team and administration to monitor client/family payer's satisfaction. Evaluates all relevant data and obtains information while carefully considering the facts surrounding the case. Assist with the implementation of a comprehensive program of utilization management to support all health care services provided by the Cleveland Health Network Managed Care Organization Other duties as assigned.

Education: BSN from an accredited school of professional nursing or Masters Degree in one of the Behavioral Sciences

Experience: Minimum of one year experience in a managed care setting, previous work with network providers, and a minimum of five years clincial experience

Licensure/Certification/Registration: Current state licensure as an RN, or Licensure in counseling, LPCC, Social Work, LISW.

Physical Requirements: Requires extensive sitting and dexterity to perform work on a PC. Requires standing and walking for extended periods of time. Good visual acuity through normal or corrected vision. Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible. NOTE: The constant stress and strain of maintaining a production rate pace, especially in an industrial setting, can be and is physically demanding of a worker even though the amount of force exerted is negligible.
