Summary: Supports the physician and interdisciplinary team in facilitating patient care, to enhance the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payors. Integrates and coordinates utilization management, care facilitation and discharge planning functions.

Job Responsibilities: Facilitates pre-certification and payor authorization processes. Facilitates the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement Understands process improvement methodologies in evaluating outcomes of care. Ensures compliance with regulatory requirements. Supports clinical documentation efforts. Coordinates communication with physicians. Other duties as assigned.

Education: Registered Nurse; BSN preferred (Bachelor in related field will be considered but not required).

Experience: Three to five years acute med/surg experience required. If role is within behavior health area, behavior health experience preferred.

Licensure/Certification/Registration: Current state valid license to practice as a Registered Nurse (RN). Professional certification as a Case Manager (CCM) required within one year of eligibility.

Physical Requirements: Requires direct patient care. Requires extensive walking, telephone use, bending and reaching. Must be able to work well under pressure and maintain professional demeanor under adverse conditions. Extensive reading, writing, use of pagers, fax machines, computers and copiers. Sedentary Work – Exerting up to 10 pounds of force occasionally (Occasionally: active or condition exists up to 1/3of the time) and/or a negligible amount of force frequently (Frequently: activity or condition exists from 1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body. Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
