Fairview Hospital has an opportunity for an Acute Care Nurse Practitioner. The successful candidate will be provide care in an inpatient Critical Care/ICU setting to an adult population. This position currently requires weekends, holidays, and call. Preferred experience includes; one year as an NP preferred, but new graduates may apply, and two years clinical experience with ED, ICU, or internal medicine experience. Certification from an Acute Care program and current Ohio CTP or CTP-E is required.

Successful orientation will be determined by the experience level of the NP. A 75-day review of performance will occur to evaluate need for continued orientation or other planning.

Additional Job Duties and Responsibilities related to this position include:

Assess health status by obtaining relevant history and physical

Develops a plan of care in collaboration with attending ICU physician intensivists

Documents findings and notes in the medical record

Interprets laboratory and diagnostic testing and orders appropriate interventions

Develops working diagnosis and problem lists

Manages medical issues and consults appropriate services as warranted by the patient condition.

May perform procedures that are reflected in privileging such as line placements.

May prescribe and monitor medications as allowed by prescriptive authority through Ohio Board of Nursing and Cleveland Clinic policy

Participates in the informed consent process and patient education.

Collaborates with nursing, case management, physical and occupational therapy, pharmacy, and other members of the healthcare team to ensure high quality patient care.

Facilitate the discharge process and discharge planning, write discharge summaries as needed.

Bills appropriately for all services rendered

Summary: Nationally certified licensed healthcare provider who provides primary and/or specialty nursing and/or medical care in a variety of healthcare settings. Functions in collaboration with a physician according to a standard care arrangement and specialty certification. Practice emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems. Certifications may include: acute care, adult, family, gerontology, psych and mental health, neonatology,pediatrics and women's health.

Job Responsibilities: Assess health status by obtaining relevant health and medical history. Performs a physical exam based upon the age and patient history as per the Standard Care Arrangement with a collaborating physician. Identifies significant health and medical risk factors. Orders, interprets and evaluates laboratory test and other diagnostic studies to assess patient's clinical problems and health care needs. Develops a diagnosis by analyzing and synthesizing collected data. In collaboration with the physician, develops a treatment plan along with the patient and significant others. Performs diagnostic or therapeutic interventions consistent with certification, law and Cleveland Clinic privileges. May prescribe and monitor medications as allowed by prescriptive authority through Ohio Board of Nursing and CCF policy. Develops patient education plan. Participates in medical team rounds; collaborates with nursing, medical and other healthcare team members regularly to ensure quality patient care. May perform admission history & physical, document progress notes, prepare discharge summaries as required. Assists with and facilitates discharge planning. Other duties as assigned.

Education: Master's or Doctoral degree in Nursing preferred.

Experience: Two years clinical nursing experience preferred.

Licensure/Certification/Registration: Current Licensed as Registered Nurse (RN) in State of Ohio, national certification as nurse practitioner (CNP) by nationally accredited organization, current certificate of authority by Ohio Board of Nursing, certificate to prescribe (CTP) preferred. Basic Life Support (BLS) Healthcare Provider required; Advanced Cardiac Life Support (ACLS) or Pediatric Advanced Life Support (PALS) may be required based upon work setting.

Physical Requirements: Typical physical demands include the ability to walk and stand for long periods of time. Manual and finger dexterity and eye/hand coordination to perform physical examinations. Requires corrected vision and hearing to a normal range. Occasionally lifts and carries items weighing up to 50 pounds. May require exposure to communicable diseases and/or body fluids.
