Summary: Provides physician and interdisciplinary team support; facilitates patient care, enhances the quality of clinical outcomes and patient satisfaction. Provides timely and accurate information to payors. Coordinates utilization management, care facilitation and discharge planning functions. Accountable for designated patient caseload. Plans effectively to meet patient needs, manages the length of stay and promotes efficient utilization of resources.

Job Responsibilities: Facilitates pre-certification and payor authorization processes. Facilitates the collaborative management of patient care across the continuum, intervenes to remove barriers to timely and efficient care delivery and reimbursement. Understands process improvement methodologies in evaluating outcomes of care. Understands and supports clinical documentation efforts. Coordinates communication with physicians. Demonstrates compliance with regulatory requirements Other duties as assigned.

Education: Registered Nurse; Bachelor's Degree in Nursing preferred. Bachelor's in related field will be considered but not required.

Experience: Minimum of three years acute medical or surgical experience required.

Licensure/Certification/Registration: Current valid state Registered Nurse (RN) license. Professional certification as a Case Manager (CCM) required within one year of eligibility.

Physical Requirements: Requires direct patient care. Requires frequent walking, telephone use, bending, reaching and occasional lifting items up to 25 pounds. Extensive reading, writing, use of pagers, fax machines, computers, and copiers. Sedentary Work – Exerting up to 10 pounds of force occasionally (Occasionally: active or condition exists up to 1/3of the time) and/or a negligible amount of force frequently (Frequently: activity or condition exists from 1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body. Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
