Home Care of Lake County is a Medicare-certified skilled care agency providing exceptional home health care services in Lake County and surrounding territories. We are actively recruiting a Supervisor of Skilled Care for our non-profit agency.
Qualifications:
- RN, BSN preferred
- Experience in patient nursing, management, supervising, administration, OASIS, SOC, RECERTS, referrals, chart audits, Medicare & Medicaid
- Strong communication skills
- Adherence to policy & procedures
- Best care practices
