GENERAL SUMMARY:

Works under the supervision and direction of the registered nurses in the Central Admissions and Telehealth department, Center for Home Care and Community Rehabilitation. Serves as an on-site resource for nurses, physicians, hospital case managers and others regarding home care specialty programs. Responsible for building and sustaining effective and collaborative relationships with individuals involved in the discharge planning process. Assists the hospital case management team and others involved in discharge planning in the identification of appropriate candidates for specialty home care service programs and provides essential information and education for patients, family members and others. Explains the purpose for and benefits of specialty home care programs; answers questions and addresses concerns; obtains applicable consents and registers the patient in the home care database system. Serves as a liaison between the hospital case manager and home care services central admissions team to ensure a smooth transition home for the patient and that the patient's discharge needs are being met. Closes the loop with appropriate patient follow-up and communications with nurses, physicians and others clinicians in the hospital unit. Participates as a member of a patient focused interdisciplinary care team. Maintains timely communication with the care team. Responsible for the initial instruction, transportation, delivery and installation of telehealth monitoring equipment for CHCCR patients.


MINIMUM QUALIFICATIONS:

Education, Knowledge, Skills and Abilities

Graduate of an accredited school of nursing. Strong verbal and written communication skills; basic computer proficiency. Demonstrated experience in public speaking, teaching and ability to do presentations desirable. Demonstrated good critical thinking and problem-solving skills.

Required Length and Type of Experience

Previous customer relations experience preferred. Minimum of 3 years nursing experience; at least 1 year in home health required.

Required Licensure, Certification or Registry

Current Ohio LPN licensure. Current CPR certification. Valid Ohio driver’s license; proof of auto insurance coverage of $100,000/$300,000.

Required Physical and Environmental Demands

Manual dexterity to operate office equipment. May require periods of sitting, standing and the ability to walk to various locations within the facility; may involve driving to home visits and to attend meetings outside the facility. Must have normal or correctable vision and must be able to clearly articulate by phone or in person. Must be able to lift at least 20 pounds.
