GENERAL SUMMARY
Coordinates, plans and conducts programs to work with transplant patients, and their families Interacts with patients individually and as a group. Develops educational materials for the program.
MINIMUM QUALIFICATIONS:
A. Education, Knowledge, Skills and Abilities
1. Graduate of an approved four (4) year program, MS or MPH preferred
2. Strong verbal and written communication skills
3. Strong public speaking skills
4. Media skills desirable.
5. Demonstrates knowledge of infection control practices.
B. Required Length and Type of Experience
1. A minimum of two (2) years experience in providing patient counseling and education.
2. Prefer experience working with transplant patients
C. Required Licensure, Certification or Registry
1. Registered Dietitian current with Commission on dietetic Registration
2. Current Florida Dietitian License-Department of Professional Regulations.
3. Certified Diabetes Educator strongly preferred; If not must agree to achieve within 2 years of hire date
4. BLS Certification required.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
1. Educates patients and their families regarding the nutritional and behavioral aspects of their medical conditions.
2. Develops and/or implements new approaches to care for the diabetic patient.
3. Provides care appropriate to condition and age of the patient
4. Documents assessment and plan of care for the patient in the medical record.
5. Reassesses and motivates patient to improve their nutritional health via follow-up appointments.
6. Provides feedback to physicians in a timely fashion.
7. Develops and implements a plan of care for complex patients or those at high risk for complications.
8. Plans and coordinates educational classes.
9. Organizes and participate in support groups.
10. Coordinates various public speaking and media appearances with the marketing staff.
11. Provides and coordinates education programs with cardiac, kidney and liver transplant patients rehab.
12. Develops educational materials for use with patients.
13. Keeps literature and material current.
14. Evaluates publications available for patient use.
15. Demonstrates flexibility, is organized and functions well in stressful situations
16. Incorporates knowledge of current clinical trends, concepts and technology in making sound decisions/judgments and resolving patient’s nutritional problems using scientific methods and intervention strategies.
17. Collaborates with other specialties to ensure interdisciplinary management and continuity of care for both inpatients and outpatients.
18. Initiates and facilitates patient care conferences and case reviews.
19. Provides and coordinates discharge planning with other members of the healthcare team.
20. Produces required reports in a timely basis.
21. Adheres to the National Patient Safety Goals as outlined by JCAHO.
22. Performs any other duties as assigned.
