- 2 years clinical experience required
GENERAL SUMMARY:
Provides skilled nursing services to assigned patients under the supervision of a Registered Nurse and in accordance with the agency policies and procedures and the physician care plan. Maintains timely communications with the primary RN and others involved in the care of the patient. Participates as a member of a patient -focused interdisciplinary care team.
MINIMUM QUALIFICATIONS:
Education, Knowledge, Skills and Abilities
Graduate of an accredited school of practical nursing.
Required Length and Type of Experience
Minimum two years clinical experience as a LPN in an acute medical-surgical setting.
Required Licensure, Certification or Registry
Current licensure as a LPN in the State of Ohio. Current CPR Certification.Valid Ohio Driver’s license upon hire; per policy 8-175 thereafter. Proof of automobile insurance coverage with $100,000/$300,000 coverage, upon hire; per policy 8-175 thereafter. Hospice full time and part time LPNs are required to obtain the Hospice and Palliative Care Nurse Certification within three years of hire date
Required Physical and Environmental Demands
Must be ambulatory and able to drive to patients’ home. Must be able to climb stairs, pull, push, and facilitate movement of the patient. Must regularly lift and/or move up to 25 pounds, frequently lift and/or move up to 50 pounds and occasionally lift and/or move more than 100 pounds. Must be able to read, write and communicate with patients. Must have correctable vision. Must have correctable hearing to be able to hear normal conversations. Must be able to work in settings with infectious diseases.
